How to refer to Home Care:

Contact the Vitalis Home Care Manager on (0490) 521 906 to discuss any new patient referrals.

IMPORTANT:
Please download and complete the Referral Form.
The Vitalis Home Care Manager will then be in contact with you regarding next steps.

How to refer to Hospital In The Home (HITH):

Contact the Vitalis HITH Manager on (0451) 439 322 for information on participating private health funds, funding eligibility, current geographical coverage, or to discuss any new patient referrals.

IMPORTANT:
Please download and complete the Referral Form.
The Vitalis HITH Manager will then be in contact with you regarding next steps.

REFERRALS CAN BE SENT VIA:

  • Patients must be clinically stable and in a safe home environment to be eligible for admission to Vitalis HITH.
  • Patients can be admitted under the care of their treating doctor, or can be admitted under Vitalis HITH’s own GP/Emergency Doctors.
  • Treating doctors are consulted and provided with regular updates.

How to refer to Chemotherapy In The Home (CITH):

Contact the Vitalis HITH Manager on (0451) 439 322 for information on participating private health funds, funding eligibility, current geographical coverage, or to discuss any new patient referrals.

The Vitalis HITH Manager will then be in contact with you regarding next steps.

Contact REHAB+ on (02) 9191 0221 for information on participating private health funds, current geographical coverage, or to discuss any new patient referrals.

IMPORTANT:
Please download and complete the Referral Form.
The Vitalis REHAB+ team will then be in contact with you regarding next steps.

REFERRALS CAN BE SENT VIA:

  • Please contact us for more information, including your patient’s eligibility for REHAB+
  • REHAB+ follows the rehabilitation program you prefer for your patients.
  • At the conclusion of the REHAB+ program and prior to your patient’s follow-up consultation with you, you will receive a detailed report showing the patient’s progress, compliance, and clinical outcomes.

How to refer to Palliative / End-of-Life Care:

Contact the Vitalis Home Care Manager on (0490) 521 906 to discuss any new patient referrals.
