This is particularly the case where patients have one or more chronic diseases, or where factors such disability, advanced age, or social isolation are present.

Recurrent and avoidable hospitalisations (including excessive length of stay) are costly for public and private health funders, for patients and their families, for employers, and for the health system in general.

RESEARCH SHOWS THAT SUPPORTING PATIENTS AND THEIR CAREGIVERS TO BE MORE INFORMED AND PROACTIVE IN MANAGING THEIR HEALTH AND CARE NEEDS REDUCES THE FREQUENCY OF AVOIDABLE HOSPITALISATIONS, AS WELL AS IMPROVES OVERALL HEALTH OUTCOMES AND MARKEDLY INCREASES PATIENT SATISFACTION.
